Sgt pay from May 1, 1862-June 30, two months at $17.00 = $34, deduct advances of $20= $58.00.

Sergeant on list of casualties in action at Garnetts Farm and Malvern Hill, 27 June and July 1, 1862, wounded.

5 May 1863, Albany. To Capt Comsy CSA, Columbus GA.  WIll please pay to Sgt J T Belk of my Co I, 2nd Regt Vols commutation of rations from 10 July 1862-1 Apr 1863, J T Maddux $195.00.

Sergeant at General Hospital at Emory, Washington Co VA; detailed by Genl Longstreet Oct 29, 1864 as nurse.  vs (my note: Vulnus Sclopeticum -gunshot wound)  left hand.

Sergeant at General Hospital, Emory, VA.  Oct 1864. Nurse detailed by General Longstreed 29 Oct 1864, VS (my note - see above) left hand, destroying use of fingers.

Muster Nov/Dec 1864, absent, Detached on Hospital duty at E & H Colledge VA.

Medical Directors office, Dec 3, 1864-detail, report to commdt of post Lynchburg, VA.

Sergeant, receiving and wayside hospital (General Hospital #9), Richmond VA.  Admitted 25 Mar 1865, detailed same on duty Mar 14, 1865.
